REGISTRATION FORM FOR MARCH 2009 WORKSHOPS AND INDIVIDUAL SESSIONS:
You will be placed in a double room unless you choose the single room
upgrade. Single room upgrades are $30/night & are limited.

Please let us know who you will be rooming with in the box below. Please
note that none of the rooms have private baths.  Accommodations are clean
college dorm style. If you need a room-mate assigned, put that in the box
below:
OPTIONS:
Full Program, Oct 1-5: Gathering of Master Shamans, 4n/12 meals  per person in double room. : $1250 (includes an $50
discount)  shared double. If you an Earthkeepers' alumnae, please take off an additional $25 for a total of $12
25.

Oct 1-3:"Seeing Auric Fields" Workshop w/Brian Osborne and additional Ceremony with Don Martin: 2n/6meals per
person double room: $650

Oct 3-5: Advanced Shaman Workshop with Don Martin& The Twins and Don Marco: 2n/6meals per person double room:
$650
Offered evenings Oct 1,2,3. Sacred Mesada San Pedro Ceremony(aprox 2 1/2 hours): $190 for workshop
participants/$205 for non-participants. Please note the evening you prefer. The ceremony will first be filled for Saturday, and
then days added as needed. In past years all evenings sold out, so you will likely get your preferred evening if you register early.

the time you register, you
will be notified and
refunded your single
room supplement fee.

We have a limited number of 30 minute private
sessions open to workshop attendees. These are
$110 with Don Martin , The Twins & Marco. Please
contact us directly at
info@earthkeepersfoundation.com if you want one
of these sessions.
We will contact you with available
times for your individual session.
These will be on a first come first
serve basis. If they are full, you will
be put on a waiting list refunded this
portion of your deposit.
When you have filled out this form,

Please remember your space is not held until
PAYMENT has been received.
You will need to bring your signed registration
agreements in order to attend.

For Individual Sessions:
Deposits are non-refundable. If you wish to transfer
your space, you must contact
info@earthkeepersfoundation.com for approval.
In the event of medical emergency please contact
info@earthkeepersfoundation.com  for our refund
policy.
Clients who forget their individual session
appointments or cancel on short notice will be
required to pay for the missed visit.  We require at
least seven days cancellation notice.  Please
understand that we allow from 30 to 90 minutes for
each visit, and a missed appointment could have
gone to a client on the waiting list

REFUNDS (NOT including $35 processing fee) are
ONLY given if event is cancelled or in the event of
medical or family emergency. Please contact us in
these cases. Tickets are transferable only on a case
